Health Information Management Specialist/ Medical Records Clerk/Scanner Department:
Medical Records- HIMS In this position, you are responsible for maintaining Electronic Health Records (EHR), ensuring that all pertinent information is properly scanned into each record.
This is a full time, in person position at a multi-specialty medical clinic.
As this is a position in healthcare, all applicants need to be fully immunized for both Influenza and COVID-19.
Essential Job functions:
Imports medical records and documents Exports medical records and documents to the appropriate entities Completes necessary paperwork for medical record request and legal subpoenas Accesses the appropriate websites for the import and export of medical records Gives updates to outside medical record companies and organizations seeking medical records.
Checks failed faxes, finds and replaces incorrect numbers Answers the medical record phone in a timely manner Assist patients, staff, and outside services in requesting medical records Assist patients with Follow My Health patient portal Corrects incorrect information for referral caregivers and patient demographic information In the future:
manages EPHI import/ direct messaging.
Scans documents into EHR in a timely fashion accurately:
Scans 90% of all documents the same day Accurately assigns:
patient name, date of service, category of service and if review is needed - the physician Checks input manager for incoming faxes frequently (every 30 min) to identify incoming faxes and directs the fax to the appropriate printer or scans them directly into EHR - to the physician for review Prints documents:
that require a physician's signature or a form that must be completed - (e.
g.
Home health care, Skilled Nursing facility (SNF) forms and DME or other order forms All Medication refill request or prior auth forms INR/PT lab results Prints to the appropriate printer.
Is aware of the physicians work schedules and faxes those documents that require printing to the office in which the physician works on the day the fax is received.
Scans directly into EHR for review by the physicians - all other medical records not listed in the to print section above Communicates consistently with supervisor:
notifies supervisor if timeliness goals cannot be met or if documents cannot be identified.
Asks questions if unsure of category assignment.
Corrects all documents that are rejected by the physician reviewer and returned to the input list - the same day.
Reads and responds to email at least 2 x day.
Responds to and sends emails accurately using email type, selecting the patient account so that patient information is displayed for the receiver.
Sends email message to all MA's and avoid use of written messages whenever possible.
Manages time and attendance using punch clock and online program.
Takes appropriate breaks (15 very 4 hours) and lunch (1 hour) are taken away from workstation or clinical areas.
Monitors total hours and punches using on line software and send a missing punch message to the Administrator for any missing punches.
Approves time card as the end of the pay period.
Other duties:
Assists other staff as necessary.
Performs other work related duties as assigned by the MD or Administrator Reports any unsafe condition in office to the Administrator Has working knowledge of all reference materials:
written, online and server desk top information.
Notifies Administrator of need to update resources.
Maintains professional and productive communication with all customers, outside MD staff, hospitals, staff, MDs, and the Administrator.
All communication is clear, accurate, concise and appropriate for the situation, including:
Telephone and oral communications, written communication including email, documentation in EMR and Pro PM.
Ensure that the window in the Medical Records room is locked at the end of the day.

Medical records experience, Medical terminology, can type at least 30 words per minute (see note at end), uses adequate oral and written English grammar and spelling.
